Before Your First Appointment

Save Time – Complete Your Forms in Advance

To streamline your visit, we will email you intake forms, consent documents, and functional outcome measures. Completing these forms ahead of time allows us to start your treatment promptly and efficiently.

What to Bring

To ensure a seamless check-in, please bring the following:

  • Photo identification (driver’s license, state ID, school ID, or passport)
  • Prescription or doctor’s referral (if applicable)
  • Insurance card

As part of our Identity Theft Prevention Program, we require a valid photo ID at registration. Your cooperation helps us keep your information secure.

When to Arrive

We recommend arriving 15 minutes before your scheduled appointment to complete any remaining paperwork. If you’ve completed your forms in advance, you’ll be ready to begin your session right away.

What to Expect During Your First Appointment

Your first appointment will last approximately one hour and includes a comprehensive evaluation followed by your initial treatment with a licensed physical therapist.

  • Our Patient Services Coordinator will help you schedule follow-up sessions at times that work best for you. For added convenience, you can also book appointments from your mobile phone and check in using our self-service kiosk at the clinic.
